How to apply

You can apply for a personal licence if you:

  • are 18 years old or over
  • are ordinarily resident in Lichfield District
  • hold an accredited licensing qualification or are able to provide proof you are a person of prescribed description
  • have the right to work in the UK within licensable activity. Section 192A of the Licensing Act 2003 defines ‘entitlement to work’ for the purposes of the Act.
  • have not forfeited a personal licence in the previous five years

To apply you must complete the following:

Disclosure of convictions and civil penalties declaration form

You must also provide:

  • the original or certified copy of your licensing qualification
  • basic DBS certificate dated no older than 1 month at the time of your application
  • two photographs of yourself, one which is endorsed as a true likeness of you by a solicitor or notary, a person of standing in the community, or any individual with a professional qualification.
  • proof of your right to work
  • proof of payment.
